Evidence of Insurability (EOI) Requirements
Voluntary Life: EOI is required if you are requesting coverage for an amount that exceeds $150,000. If you are not currently enrolled in the plan, or you are currently enrolled in the plan for a benefit less than $150,000, you may request coverage up to $150,000 without providing EOI.
STD and LTD: EOI is required if you did not elect this coverage when you were first eligible as a new hire.
If you do not submit the EOI form to The Hartford, you will not be covered under the benefit.
The EOI form (or statement of health) and directions on how to submit to The Hartford can be found on the ADP site.
